At the Vatican you still need your shoulders covered and your head covered.

This is not true. I've attended a mass led by the Pope and no one in there had their head covered. It was summer and many were in summer dresses.

Yes, I have as well - it was beautiful.   However, for a private meeting it is a little different, I think.  I have never had one, but I have traveled with someone who did.  She did wear dark clothes and head covering for that meeting.  I thought it was very respectful.

As far as going in to St. Peter's Basilica the dress code is much less strict than it used to be.  I have gone in in long jeans and 3/4 sleeved shirt with a scarf around shoulders many times.

In the 60s I have been told, the nuns of the Vatican stood at the entrance to the cathedral and their interpretations were very strict.  However, now the Swiss Guards are at the entrance and the dress code is not so strict.

I'm going again in March, I always, always go to see the Pieta, it transfixes me every time.

I've been blessed to visit the Vatican a number of times.  I've seen men asked to remove their head coverings but I've never seen a lady asked to cover her head.

In some of the Italian Cathedrals we've visited, they have disposable shoulder and leg coverings for summer tourists.
