Question about a doctors responsibility

If a doctor asks you something like "you aren't thinking of harming yourself, are you?" and you say yes, what is the doctor required to do with that information? Can he force you to be committed to a mental hospital or what? Does it depend on the state you live in? I'd appreciate your responses. Thanks.
