If your numbers are that high and the doctor has never addressed them then it's time for you to find a new doctor.
Why bother ordering the bloodwork if he/she isn't going to talk about them.

Thoughts on the fluctuations? I go along with @SilleeMee comments. Could be several factors. Different labs do produce different readings.
Were all your lipid tests done on the fasting protocol? You mentioned you haven't made any eating or lifestyle changes. Maybe shaking something up a bit and seeing what happens might be something to think about.
Like others have said, and your are familiar with. I myself pay little attention to my Total Readings because I see the other parts as being much more important in my overall lipid status.

@hckynutjohn Thanks for your thoughts! Greatly appreciated you sharing your thoughts instead of the usual "talk to your doctor".
All four readings were done on a fasting protocol.

I don't post much on the forum but I do regularly come here to get my daily news.
I was hesitant to post this because of the usual "discuss with your doctor" answers.
I am not worried about my cholesterol level, just CURIOUS. I'm not surprised my cholesterol level is high because of my diet.
When my cholesterol was 264 my PCP mentioned it was a bit high and we discussed a healthier diet. When it was 344 again discussed my diet and that's when I first heard about my ASCVD risk 1.0 (I believe). When it went to 374 my PCP ordered a carotid ultrasound and if it showed plaque she would have to put me on meds. No plaque on ultrasound. Again we discussed healthier diet.
This last reading was just a few days ago. I have a phone appointment next week to discuss my labs. I was just surprised by how my cholesterol level came to to 264 from 374.
Breakdown of my 374 level:
HDL 141
LDL 227
Triglyceride 30

Clinically there isn't much of a significant difference between 264 and 374 for total cholesterol numbers.
High total cholesterol levels are any numbers above 240.

@Runner26 as an RN I can say that your levels are not normal, your levels will flufatuate with weight, diet, age, and life style- however if you have pre diabetes or diabetes this will affect your levels also- your doctor needs to order a complete lipid panel to see what your LDL, HDL and ratios are- there are supplements you can take that are not statins but please get this checked out- I don't know your age but it can be corrected- and some of it can be genetic also- good luck and stay well

Genetics, diet and age are all contributing factors. Yours continues to be in the very dangerous range so those fluctuations are irrelevant. I hope you are working with your doctor to lower your cholesterol.

I'm of the belief that we must own our personal health issues. That is to say educate ourselves about our conditions and be a good savvy patient.
But at some point the self education of one's health concern should shift to finding the physician that will treat your out of the normal range cholesterol findings.
My advice is to stop trying to explain it away and find a good doctor.

I admit to being a renegade to regular bloodwork. Unless I am in an active state of white cells vs. red blood cells and vice versa, I pay little attention to bloodwork. As long as I can go out and do my own hurricane cleanup, as I have always been able to do, short of large tree work, I have said to heck with bloodwork.
I am in the process of cleaning up my one acre property from hurricane Ian. As long as I can keep going with this, I don't need bloodwork to tell me how I am doing.
This is strictly my personal account.

I would change that unhealthy diet immediately.

"This is strictly my personal account."
Not sure what you mean with the above quoted sentence. I will just comment on the paragraphs you said above that particular sentence.
How one chooses to gage their physical or mental health, along with how they interpret their physical fitness, is certainly up to them.
From my own "personal account" I can site many of my physical abilities. Those abilities did a good job of covering up what would have turned out to be a death sentence, except!
Part if my personal account included procedures, including blood readings, which took a sure death sentence, and prolonged my life. Not just in an existing state, but the very active state to which I have been accustomed to, much of my long adult life.
At my present age I will accept the physiological things I cannot change, along with doing the same with what might be. About 20 years ago I would not have accepted what I just outlined above.
I have always used proven Physiological Studies, some which included my body as part of 2 studies, to gauge my Physical Fitness levels and abilities.
These however have/had no direct connection to my Physical Health. Had I used that method and applied my Physical Fitness Abilities to my Health? Once again, that would have been my death sentence to myself.
I have no comment on how you choose to gage any facet of either physical or mental health, and/or physical fitness. My above written(typed)words are from my life experiences. How you or others view them is each individual's choice.
