I've lost over 70 pounds and kept it off over the last two years with calorie/portion control, logging everything I eat and exercise. I make 90% of my food from scratch and on those times, like over the holidays that I go off, I've never gained more than 3 lbs and that was mostly water weight that came off in a week.
