for a family member that just had her fourth baby boy. All the children are from 6 years and under.
I doubt she needs anything having four boys, so I need some ideas.

barefoot dreams stroller blanket with a lovie.
you didnt really mention how much you wanted to spend, but i love barefoot dreams for baby gifts. it is something she will use for a long time.

Even though this mom has all boys the things she has may be worn out. New items are always welcome. I usually give several onesies in increasingly larger sizes, some receiving
blankets, box of diapers and baby wipes, etc.
They will all get used.

FOUR kids under six years old ???
With all the hand-me-downs from siblings, the new tyke already has a decent wardrobe, I'm guessing.
Forget a baby gift ... get a MOMMY GIFT ....
Coupons for babysitting from a babysitter or female family members?
How about a gift certificate from a spa for a massage?
Don't know if that might be more than you want to spend but it might prompt other ideas .... manicure and pedicure?

If you are capable and it is within your price range, how about a gift of baby-sitting, along with a gift card to a restaurant? Or a good meal sent in? A small personal gift just for the baby, along with these possible mom gifts?

@Sunshine Kate. Sounds like someone I know except baby #4 is due in March. Oldest just turned six. I think she's out of her mind.
She recently drove alone from NJ to Disney World in Florida with the kids, did the park features then drove home. Hubby flew as he works on commission and couldn't take days off to drive.

After many years of Fertility treatments my Son and daughter in law were blessed with two daughters. One just turned 3 and the other one is 10 months. She is currently 4 months pregnant with twins conceived naturally. So they will have 4 kids aged 3 and under! What a shocking turn of events ! They thought their family was complete as one was delivered at 30 weeks and one at 35 weeks due to complications. Both are healthy! So far this pregnancy is going well!

@Sunshine Kate I agree with a gift for Mom. You can always get something small for baby. Diapers always welcome unless she uses a diaper service. Merry Christmas!
