How do I transfer to my iTunes Library???

I have the third update to iOS on my iPhone 4 ready to download. But...I get a pop-up that reads:

"There are purchased items on the iPhone that have no been transferred to your iTunes Library. You should transfer these items to your iTunes library before updating this iPhone. Are you sure you want to continue?

Well, I backed it up, it even reads, "Last backed-up to iCloud today 4:25pm". I clicked on "Back Up Now" - so it went thru it again but when I clicked on "Update", I get the same pop-up as before.

So - how do I transfer these items to my iTunes library??? Isn't that what a Back-up is for??! Please let me know what I should be doing next that I'm obviously not doing!!!
