Your kitty is picking up odors of other animals from the vet's office, and that is why your other cat doesn't recognize him.
This would happen to us too, and what I would do is take some X-O odor neutralizer and use some of it to wipe the kitty down who went to the vet's.
This would do the trick, and the other cat would stop hissing and swatting at the one who was at the vet's.
