We each have "fun money" that we budget for and use at our discretion. We've been married a relatively short period of time compared to many posters (almost 5 years), but we haven't had one single fight about money. I think it's because we respect one another, are comfortable financially, and have the same goals for our future -- financial and otherwise -- so it makes it easy for us.

Joint. We only have a checking account, a small amount in his credit union. My name is on my dad's account as well as the title of his car.

Both.
Joint account where all money goes for all household expenses etc.
He has a separate account just for ebay purchases and sales, as he likes to dabble there, and I don't want pay pal in my regular accounts.
I have a separate checking account that gets funded just for property taxes. I find it easier to save for that if I have a separate account just for that. He wasn't available the day I opened it, so I just put it in my name only.
Our separate accounts never have much money in them, and are marked for one special thing.

We never sat down and decided exactly who pays for what, but we use the joint account for the house, utilities, insurance...other shared expenses.
We use our own money for our own clothing, gifts for our friends, different charities that we each like.
And for those of us who file a joint tax return....yes of course we know what the other has when we go over the year's end financials.
I believe there are some spouses who file as individuals....but I think it's rare because it costs more to do so.

VanSleepy, since you mentioned kids, ours are grown, but I forgot to mention that the joint account also covered all of their expenses. But I often used my personal account to give them extra special birthday or Christmas gifts! Because I wanted to....
