It can go either way.  If there is NO real connection between the two parents, the surrogate's own egg may be used.  Then they just do artificial insemination.  But if there is any relation of the surrogate, they may do complete IVF with a donor egg.  Many surrogate pregnancies are from just artificial insemination.  IVF if way more complex with a donor egg.
