I could not agree more. I went to a strip mall salon and ended up with toe nail fungus. Since then I've only gone to the solo nail person at my hair salon. I bring my own soaking bowl and implements.
The strip mall salon I went to was new, and I thought those big massage chairs with whirlpools would be so relaxing. After my experience and watching many news stories about them, I'd never put my feet into one again.
Ditto. I got an infected toe from a strip mall nail salon. I had to get it lanced and drained, which was quite painful. Unfortunately, you need to be proactive and take responsibility for the sanitation because a lot of the cheap nail places don't. Even with the more expensive places, I would ask when you book your appointment about their sanitation practices and hold them to it.
I got infected, but I think mine was probably from the gym and not a nail salon. But mine was painful under my big toes so I had to take the Lamisil. I tolerated it very well, but the after effects were brutal. It strips your entire body of good bacterial and I got every known ailment under the sun. Took a year to get my pH and good bacteria back in check.

I damaged my nails from over filing and had to dial back my manicures and pedicures to give them time to heal. It was tough.

I've been getting monthly pedicures for at least 25 years. Knock on wood - I have never had one issue with fungus or infection.

Does the technician clip or file toenails? Usually, but you can request that they not do this, if you want. It's included in the fee and I wouldn't expect a discount if you skip this step.
What type of shoe/ sandal to wear (so I don't ruin polish on my way home)? Flip flops that you can easily get your toes into.
Does the technician use different instruments (cleaned or sterilized) for each customer? By law, yes. If you have questions, ask the person. If you see anything you feel uncomfortable with, ask about it and don't be afraid to leave if you think something is wrong.
Is a 20% tip appropriate? Yes.
