I found this online. I haven't had the need to use it yet, so I can't vouch for it as to whether it works or not.
Anyway, it wouldn't hurt to try if you need to.

This has been floating around the internet for a while. Not sure if it's a magic bullet but it seems factual.
If a doctor ordered a procedure for me, it was usually the doctor's office who jumped through all the hoops to get it paid. This seems like a last resort.

Having worked at a major insurer, I can assure you that decisions aren't made by HS clerks. Insurers hire RNs, PAs and NPs to perform these functions.
All insurance companies, by law, have formal appeals procedures.Most have a deadline to appeal.
Make sure to let the doctor/hospital know if you are appealing, so your bill won't go to collections.
The article you referenced is pretty confrontational - I doubt it will get you very far.
It doesn't address the problem - just tries to bully the insurer.

WHAT'S TRUE: HIPAA laws entitle patients to access to their medical records (with limited exemptions); insurers unable to document adherence to healthcare laws could conceivably reverse a denial decision to avoid hassle.
WHAT'S FALSE: Federal laws mandate all documentation stipulations in the meme; denials must be decided by a physician or specialist.
WHAT'S UNDETERMINED: Whether many assertions in the meme truly are rights mandated by federal law; whether inherent legal flexibility to those laws provided large enough loopholes to render its claims meaningless.

This is another case of someone giving credence to something just because it is online.
This is why, in 2016, conspiracy theories and junk science run rampant, and why, at a time when we have the culination of the world's knowledge at our fingertips, we are, perhaps, largely ignorant and anti-intellectual.

Every insurance company has an appeal process if you apply for a medical procedure ,xray.mri.new medicine ,or operation .etc. If your insurance company says :no
then you have the right to appeal the decision , Not all companies will tell you about the appeal process and some will even deny that there is one , But they allhave them so be pushy and know your rights , APPEAL PROCESS

Does this include portions of bloodwork? Several months ago my doctor ordered a list of bloodwork he wanted me get. It was a long list, however, not all of the tests were covered. I received a bill which showed all the ones that were not covered which I would be responsible to pay adding up to over three hundred dollars.
I have Medicare and Aetna Supplement Plan F. I contacted Aetna about this and they said if Medicare denies it they don't pay. They pay what is still owed after Medicare pays. Hundreds of dollars is a lot more than I have paid for any medical expense for years.
My broken hip surgery and one month in rehab and home physical therapy visits for months did not cost one penny. Between Medicare and Aetna everything was paid one hundred percent. I would love to get someone to kick in for that bloodwork.
