I did do one QVC survey awhile back.  Based on the questions, I think the point of the survey was to lead me to get a Q card.  NOT doing that.  The survey was a waste of my time.

If a corporation is interested enough in improving their operation in any manner by paying to have a survey done, I'm always eager to help.  Since they're usually anonymous, "SCAM" would be the furthest thing from my mind.  How could asking what kind of cell phone you have possibly be a scam?  Some people no longer use common sense and consider everything a possible scam.  It pays to be cautious, no doubt, but people have become paranoid beyond reason.
