Question about KitchenAid $50 Rebate for 6Qt. Pro

I purchased the 6 Qt Pro Bowl lift mixer back when it was on special pricing. At the time there was a $50 Rebate Offer on purchases made between June 30 to August 31 for Models KP26M, KP26N, KL26, KSM6573; KSM6521 for select 5.5 or 6 Quart. According to the paperwork, my mixer is the KP26M model which is listed on the form but I got a card back yesterday not questioning the model number but rather the UPC. I was told that I did not include any valid original KitchenAid UPC. Well, maybe that is questioning the model number but I can submit the missing/valid item no later than 10/16. Naturally I thought maybe I had keep the original and sent the copy by mistake but that wasn't the case. When I got home, I checked the box, there isn't another UPC to be found. I shall give them a call Monday but I was curious if anyone had experienced this same issue. UPC.
