Margaritaville Mixed Drink Maker Rebate

I purchased the Margaritaville Mixed Drink Maker last week (June 24) and there was supposed to be a $50 mail-in rebate with the offer. Does anyone know where the rebate is located? I looked through all the paperwork and see no form for a rebate. I tried the website for Margaritaville and don't see any rebate offers there either. Thanks!
