Took my two cats to the vets today for shots and teeth cleaning. We have used this vet for 5 years now. It has probably been a year since I was last there. THey handed me a "client registration" form as I was leaving and told me that they had changed their existing forms so would I please fill out the new one and bring it with me when I pick the cats up. When I got home and looked at it in addition to the usual names, street address, phone #'s, E Mail address they want both my DH's and my Social Security #'s. Why on earth would a vet want this information? I can understand a people doctor needing it for insurance claims but I will be paying when I pick the cats up. I don't think my SSN is any of their business and I am going to refuse to provide it. Any thoughts?
