Plans for your pets

I was wondering if anyone here has made arrangements for their pets should something happen to them. We have a cat, and we will likely outlive her (she's 15), but we have family who have agreed to take her in case something happens to us. I know an elderly couple (mid-70s) who are not in great health who got a puppy (a breed that will live up to 16 years). It is possible the dog will outlive them. I just wonder what happens to all of these pets when people pass away and don't have plans for anyone to care for them. It must be heartbreaking and very confusing for the animals once their owner is gone.
