This drives me crazy as well. I used to work with a guy who always wanted to "ax a pacific question" instead of "ask a specific question". How he made it to management is a mystery.
It also drives me crazy when I hear someone say "it's a mute point" instead of "it's a moot point". Or, "I could care less" instead of "I couldn't care less".

I hate it when people say "Myself and Mary" went to the store. Or, "She gave it to myself and Mary". This bothers me expecialllly when they work in a publx speeking job like telebision sails.

The one I seem to hear a lot is 'me and him went to the store'.
Really? ME went to the store? HIM went to the store? REALLY??

When DD was growing up, she picked up "on accident" from kids at a very posh school for girls. Now I ask you, how in the heck does this type of thing occur? I taught her "by accident" and still correct her to this day (she's 37! I don't want my 15 month old granddaughter learning that.). All of her friends up here in our new home town say "on accident" as well.
Re the OP's entry, I've never heard that uttered up in our neck of the woods. Perhaps it's a regional thing.
