I did for a while but packed it up. The vanity top was looking like a dental station with old and new tooth brushes.

I mostly buy those spin brushes, when not using my sonic care. They always have coupons for those. I haven't used a brush that I have to manually move up and down for quite some time, as I brush for 2 minutes.

1) spread toothpaste on bristles of new brush, 2) put brush inside mouth, 3) push back/forth.

I always wash mine. Usually using Ivory bar soap. Also, someone mentioned peroxide, which is a good idea, as some dental offices used to recommend using it, dissolved with plenty of water, as a mouth rinse. Actually, they also suggested adding a bit of mouthwash to the rinse, for better flavor. So using it to clean a new toothbrush is a good idea. p.s. The reason I wash my new toothbrushes, is because a hundred years ago (lol), I recall buying 'cheap' toothbrushes which had a terrible taste. Could have been a chemical in the plastic bristles, etc. Never actually used them, except for cleaning grout, etc. So, buying a reputable brand or well known pharmacy in-tore brand is probably a good idea, as we can return them/satisfaction guaranteed, etc. Good, safe new toothbrushes shouldn't have a chemical/bad taste.

It has never even crossed my mind to clean a brand new toothbrush. It's wrapped pretty securely, and I guess I would just assume the inside of the plastic it's in is sterile. Otherwise wouldn't it say to wash before use?

I wash all of my clothes first, as often times they have a chemical smell to them. First I air them out on a hanger, let the slight odor dissipate for a week or two, then wash. I believe most sheets and blankets have a suggesting to wash first. (maybe because of sizing sprays on the fabric(s). Some pants suggest washing first, possibly because of the darker/black dyes, as I recall. The chemical odors, I believe, it's possibly because many anti-mildew-fungal/anti-pest sprays are used on items being shipped on ships, etc. Or just maybe the stores are spraying the clothes, as an anti-moth/spider preventative. Whatever, I always wash first. I've yet to see a moth or spider, etc. on a hanging store item.

I don't leave my toothbrush out on the counter either. I have a cabinet that we store our toothbrushes in.
I've already heard that you shouldn't leave your toothbrush out on the counter if your sink is near a toilet. The spray from the flush can settle on your toothbrush.
We have a separate area for the toilet, so it's not a problem.
