While we may probably never know the cause, the unfortunate facts are they can never be cured and they get worse as they age.  I've known quite a few of these individuals in my life . . . closely, and two are not men, but women.  They are narcissists, with a completely different thought pattern than the norm.  They are born that way and will die that way, never really caring about others than themselves.

 

It's a tragedy really, for everyone close to them.  But in my opinion, mostly for them because they miss the most glorious part of life . . . loving others.
