Cat w/hot spots.

Help- my mostly outdoor cat,( who has been in all winter,) has developed what I think are hot spots on his lower back. I just noticed today a bit of missing fur and abrasions. Is there a good home remedy for this? Or is this a trip to the vet? Thanx in advance for any help.
