The pre-disposition to be empathetic is born in each of us humans in various degrees.  Then it is hopefuly taught (but often it is not).  Teaching the toddler that hitting the family dog over the head with a meat mallet hurts the dog just as it would hurt you is part of teaching that child empathy. Compassion works the same way. Without the necessary nurturing and constant teaching of that child from birth... you end up with an adult that does not understand and can't put themselves in the situation to the point of understanding.

Actually there are some conditions where people lack the appreciation of theory of mind, like in  some autism, where there's not a  remote predisposition to be empathic. "you end up with an adult that does not understand and can't put themselves in the situation to the point of understanding." with teaching.
