It's happened to me, I think after I waited awhile to actually click submit so it basically timed out.

I've tried chrome, safari and firefox. I've tried ordering from my iphone (both on wifi and cell network), computer, and my tablet. I've cleared my cache, history, cookies on each of those devices and still I cannot place an order

I never got any error messages that there was a problem with the card either. She added the expiration in, and I have been able to order since then. I guess they must have updated the website at some point and all the data did not carry over. Try checking to see if that helps.
I think that might be it, my redit cards are listed as payment options when I'm doing the check out process however I just went into my account under credit cards to check the experation date and there are no cards listed!
I added one but I don't know if the system is slow to update or if my account is cursed but it is not showing in there yet.
