Blackheads - Again

I am 61 years old and have suffered with acne prone skin (now more Rosacea) most of my life. For some reason, just these past few months, the blackheads and pores on my nose have really given me fits. I have tried several different cleansers (Biore, Murad, Replere, PTR, etc.) I have also seen my derm who gave me a cream to use at night. It just hasn't gotten better. Any recommendations?
