@Mom2Dogs you're right about not being able to ignore the pregnancies. She is very short and is pretty rotund carrying 2. They'll probably have to do a C-section.

It is tacky for the expectant mother to throw her own baby shower. Also, clients should not be invited to events meant for close friends and family. It is nothing but a gift grab.
That said, I would not go even if I was available, but send a card with a gift certificate in a reasonable amount, maybe $25 if you intend to still use her services.
She may retire for a while with two babies.
You can also just RSVP not able to attend and forget the gift if you're comfortable doing that. I turn down wedding and baby showers on the regular unless someone is special to me or related.
I am not someone's ATM.

I had identical twin girls, 57 years ago though. One thing with twins is you never, ever have enough diapers Also diaper aren't cheap. But reasonable enough for a gift, maybe a couple big pks from Costco. My family swears by Huggies. Also, bibs, and organic muslin receiving blankets they use for swaddling. Another nice gift is reading books. My granddaughter reads to my great grandson since he was born. I always give Baby Uggs as a gift. But with twins that is a lot of money. I know twins are double the cost ..right up to college, lol but double love.
ETA. I'm in California, I don't know how it is across the country, but here diapers are number 1 gift. Friends and relatives will shower them with clothes and such, but it's dialers they have to spend on every week for 2 years or more. Sometimes the new mom even asks for certain sizes. But for now you can be assured they will need. Newborn

She seems to think of you more than just a client. I'd give a $50 gift card to whatever store she's registered at.

@AuntG excellent idea! The new moms seem to prefer Target and Baby Gap. Both have great quality, even organic clothing and essentials

A diaper caddy, diapers, and wipes makes a great gift if you would like to give one.
Mom can still use the caddy for organizing when babies are older
