Just reading the first page, it appears that more people are just going to have to speak up. It's easy to say, thank you for coming by, I really need to lay down and rest quietly for awhile. I know you understand, and I'll call you next week (or whenever). If they don't know, you lead them to what you need to have them do. Some things are awkward and some people just don't know what to do. Light the path for them to follow.

That was my first thought too - Maybe the friend didn't have a problem with it. Maybe there was an earlier conversation in which they had discussed staying as long as the friend needed them, or something along those lines.
Some people like company in these situations, others don't. Some like visits that are short and sweet, others prefer the distraction of visitors. It seems that the OP is the one who had a problem with this, not the friend.
