15-20%, just like the average tip for most other services, including servers in restaurants, bartenders, hair stylists, etc.

I get a massage locally ever other week and tip $20 each time. Same at vacation resort spas. It's a little high but I enjoy the services and would rather err on the side of generosity. And the practitioners recognize that and have gone to extra lengths for me when I've had problem areas etc.
