At times I have insisted on a hard copy as well...so I could shop for the best price.  I live in a small area and have always been give the written copy.

What you're referring to is an outpatient order issued by your doctor for whatever test you need to have done.  My local hospital requires me to have that paper copy as well when I show up to register and have the test done.
