First thought is that you may have undiagnosed arthritis in several joints. If the hand pain is on the palmar side of your hand (not the top) and is at a center point near where your had bends, then mild carpal tunnel may be the culprit. With regard to your forearm/arm, make sure you have your arm bolstered: e.g., place a small pillow or rolled single sheet under your arm.
Another matter to consider is how you are holding your crochet hook. If you're holding it like a pencil, which is very common, please know that this method requires more movement of the hand, wrist, etc. However, if you use the German method, which is my preffered method, the hook is held on the top side of the hook, with the thumb on the left side of the hook and the 4 fingers on the right side of the hook. Using this method, you're going to do yourself a favor by making the yarn do the work, instead of your hand.
