I noticed the same thing when I checked in with the doctor's office. They always ask you to check in online or text before, but it does not seem to matter when I arrive, so what's the point? Like you, I have stopped checking in the previous day.

Thanks. I am tender, but okay.

 

 It wasn't a form.  It was a reminder telling me I had an appointment.  Also, was a request for me to see if my name, address, DOB and insurance was correct.

 

I get these before every appointment from all my medical providers.  None of them ever have asked any other questions.

 

Normally, I  just say all info is correct and send it back showing I saw the reminder.

 

It was weird.  I hope I don't get another one.  If I do, I will take a screen shot and show it to the office for proof.
