Tetanus shot with pertusis or diphtheria?

A while back due to a minor accident my doctor ordered a tetanus shot. I thought the shot included pertusis but when I looked through my records I found an information sheet on tetanus with diphtheria shots. Does anyone know which it might have been or could it have been either one?

If no one knows I guess I'll call my doctor's office tomorrow to find out. Thanks.
