Shoulder pain

I think I tore something in my shoulder working on my landscaping. Anyways...my question is...if its a torn cuff when do you have to have something done? Will it get worse with time or kinda heal up? When it hurts it brings tears to my eyes, but it doesnt hurt all the time.

Just thinking...I know I will need to see a dr.
