The hospital here has kind of a restaurant type menu that patients order from. Anyone who wants a plant based diet simply needs to order that way. No need for passing bills, etc.

My point is that your mention is of only one hospital. 

I have no doubt that in at least one hospital in each state it's the same way.  That doesn't guarantee the other people in all the other hospitals the same options.
