She might need to be in a hospital or some atmosphere where her actual intake & activities can be monitored. I hope she finds help before it gets worse OP.

Huge red flags are waving. The word "Critical" comes to mind.
Have you spoken to your sister or brother (parent of your niece)? Have you determined exactly what they've done to date to determine what the issue is? I would be frantic at this point, making arrangements to get my daughter into Mayo ASAP.
What is most important is that a savvy physician take the bull by the horns, hospitalize this young lady and run tests and bring in specialists until they have a diagnosis and can treat.
Waiting is not an option.

People with eating disorders can be VERY secretive. Even family members are fooled sometimes. This extreme weight loss and no period is extremely serious!!! She should be hospitalized and monitored carefully. It could mean the difference between life and death!!

If she eats a lot of fish and vegetables, why would her body be in starvation mode?
She may be eating that as a cover. I would guess bulimia.

If she's in or near Boston, she should have access to some of the best doctors in the country, if not the world - eg: Mass Gen'l, Boston Children's....... She needs to see an Endocrinologist to begin with IMO.... I don't know why someone hasn't found the root of her problem yet...... Why would people on this bulletin board have to give OP ideas???? Surely, this child is being taken to specialists?????

She has been to doctors and been tested etc but for some reason you think people on a shopping board who have no idea of her history or any information about her can diagnose her????
