As someone who had a badly broken ankle with two different bone infections that required removal of the hardware, I can say that swelling and pain could indicate infection, but that's unlikely to wax and wane.
It could also indicate something the head of infectious diseases warned me about and which I haven't experienced, an imperfect healing that would cause the kind of symptoms you describe and make it hard to walk. It's not fixable, from what this guy said, and I certainly would not tell this woman about that possibility.
She needs to have a surgeon go over fresh X-rays of the ankle to see what's going on. They may want her to go for an MRI. I'd encourage her to get the diagnostics, but would not share doomsday scenarios with her.
Thank you for being concerned enough about her to bring this here. It's great she has people in her corner.
