Evidently actual investigation isn't done prior to releasing a story.  

I just read about Joe Guidice being released from one prison and is now in another until he's deported.  The story claimed he is now in Allenwood Low prison and the report claims it's in western PA.  Well, actually, it's in the eastern part of the state.  I'm from PA originally and our niece's husband was a guard at Allenwood so I know where it is located;  just to be certain I looked at a map of PA.  If anything, the location could be described as eastern central PA, but definitely not western!  It took me 2 seconds to research that info.  

Now, I know this isn't major, but it makes one wonder what important news stories are incorrect due to inadequate research.
