Sorry, I got a phone call and missed a little bit. What did the orderly do to his brother to keep him from having to go back to the front?

They didn't show what he did, only that he was screaming and a little while later we saw him with his leg in a cast.

I was sorry to hear that the BBC canceled this show. Evidently the creator had three more seasons planned.
