@RinaRina Advantix is a topical. I'm wondering - what exactly does this "hot spot" look like on your dog?

Aww, poor baby. I had my dog to the vet today for something that's been going on with his front paw, underneath up in between his toes. I straight up asked her if it was a hot spot that he had and what a hot spot actually is because I've never seen one. His was not, it was something different.
I'm guessing, but your dog's dermatitius maybe caused by an allergy either by something ingested or by contact. It may or may not have to do with the flea pill as it sounds like he/she was kind of bombarded with a few different things all around the same time. Did you change the food he/she was used to eating? When you gave the flea pill, was that the first time he/she had that one? Were they giving it at the shelter? Maybe he/she already had some fleas, if they weren't. If it were my dog, I'd go back to Square One with the original dog food and not give any other treats or anything until that sore heals and then just add one thing at a time with a long testing period for each to see if there's a reaction. I might even give Dinovite a try. I'd also get some antibiotic ointment for that sore. Good luck as you try to get to the bottom of things.

It reads like you're doing too much all at once.
Heartworm:
I wonder whether the dog was tested for heartworm before he got his dosage. Even though i don't live in a high heartworm area, vets in my area are now beginning to test once a year. I trust your vet wouldn't have given you something she hadn't tested for.
Fleas/ticks:
So if you gave him a pill for this, then either he threw it up or else it's still preventing the fleas/ticks. I think you'd want to be sure before you begin using any topical. Before using a topical, read about the advantages and disadvantages of Advantix, Frontline, and so on. For example, the ingredients in Advantix are harmful to cats so since you have a cat, that's something to take into consideration. Some dogs also react to Advantix. Different dogs have different reactions, as you probably already know.
Dog park:
You ought to think about simply keeping him home until you get his medical issues resolved, and any other things that might come just because he is new in your household.
Food, and other items:
You've been given some good advice about keeping it simple.

@GingerPeach This was a concern of ours when we first began using it because we had cats too when we brought our dog home. What I was told is that it's fine if cats and dogs are in the same household, the two just can't be close to one another (playing, laying or sleeping together) for the first day after application. I understand your point of view and agree that not every product is good for every animal. That original info I referred to earlier about Trifexis and deaths really shook me up. I asked the vet about it and she hasn't had any occurances of those kinds of deaths in her practice. Even so, I was still too scared to use it. Fortunately, the K-9 Advantix II has given my dog excellent protection. When the weather is right you can see swarms of gnats and mosquitos hover around him but they never land. Meanwhile, I'm getting eaten alive out there! In the end, everyone has to do what they believe is right for their own pet, which method and product to use, but I do believe that protection is required and should be used year round in order to be safe, rather than sorry.

I hope you didn't misunderstand. I do not advocate being without flea/tick protection. My point was to make sure those who apply it are aware of the benefits and drawbacks of the different brands and ingredients.
(I personally do not like to put a lot of pharmaceuticals onto or into my dog, or myself, for that matter. I use the least toxic I can find. For my dog, it's Frontline, but I understand some people in some areas do not like the way Frontline takes longer to work.)
