punch recipe

Does anyone have the punch recipe from the QVC cookbook (paperback) for a banana slush punch (maybe it's pineappe slush)? The ingredients are pineapple juice, mashed bananas, ginger ale, lemon juice (not lemonade) and probably sugar and water. Someone borrowed our book and we need the measurements. Thanks so much if you can help.
