My mother and I have had several pieces remade, and I would suggest pre-made mountings if possible. Sometimes the workmanship is not what you might think it would be on custom made pieces. Some we've had made have been wonderful, some not so.

I did just get this three stone design semi mount from Gelin Abaci. You can only set gems 9 or above on the mohs scale in these tension settings. You can see their whole catalog online.

Many of the companies do have online catalogs you can look though online. If you find something there should be a dealer near you, especially Stuller who sells parts and findings to most every jeweler.

The only thing I have to add is to look extensively to find what you like. You can google rings and see thousands of designs. Make sure you go to a jeweler who does this all the time. Do not go to a mall jeweler. The cost will depend on what you choose, but please make sure you take the old setting back or sell it.

I had several items that I took the stones from and had new pieces made. It would be good to look around on the internet to see what sort of style suits you so you will have a feel for where you want to end up. Then go visit with jewelers who do this type of work. Once you see their design books you will know when they match your vision. How fun for you.

I think that an eternity band comprised of the two smaller diamonds from your original engagement ring, along with additional stones of the same size (could even be good quality CZ), would be classic.. and classy.
A deceptively-simple perfect circle.
Your could wear it either way:
*stacked next to your anniversary band
or
*on the opposite hand's ring finger
