Help with how to clean shower tile grout?

Our shower is about 23 years old and the ceiling and walls are glossy tile with small, narrow grout lines. There is alot of grout, because the tile is rather small- about 6" x 6" squares. The grout was once white, and over the years it has turned sort of yellow. This shower is really hard to clean. It is such alot of work to scrub each gout line, that I don't do it nearly enough. When you try to spray the ceiling, the spray ends up falling on you. There is no fan inside the shower-it is about 3 feet away on the ceiling of the bathroom. So the steam has to go above the shower door (an opening of about 12" by 36"). I try to run the fan for a while after using the shower- but with the way the shower is built, the air circulation leaves much to be desired.

Does anyone know a product that works really well for shower grout? I have some mildew in there too. The grout is discolored (not pure white anymore), and no matter what I use on it- it still look dingy. Using chemicals in a semi enclosed area is tough too (which this is). I would really like to clean it up, and have my hubby repair some of the grout which is a little sparce in places. Thanks to anyone who can help. I would never, never put in an all tile shower again!
