Vacuuming the Mattress

I am a neat freak and keep a clean house, but I was shocked when I vacuumed my mattress yesterday. I have a very thick, very plush mattress pad so I didn't see how my mattress could be dirty, but boy was it! If you routinely vacuum your mattress, how often do you do it? My mattress and box springs are two years old, and this was the first time I cleaned it.
