The investigation, commonplace for an unanticipated death, will look at the clinic, the procedure and what emergency procedures were followed. An 81 year old is at a much higher risk of death from anesthesia than a younger person. It would be standard procedure to, at the time she signed the consent, inform the patient of the risks involved and review her "medical clearance" e.g. is she healthy enough to tolerate general anesthesia.
No one should take the subject of general anesthesia lightly. As we age our risks of heart rhythm abnormalities, hypertension and other age related abnormalities increase. We don't know whether this was an elective procedure and why she chose to have the procedure at a clinic, not a hospital.
Truth is, we don't know nor will we know what her risk factors were nor why the clinic setting of the procedure was deemed appropriate for her age/health.

I have never been a fan of Joan Rivers but I convey heartfelt condolences to her family and all of you who were fans. The good news is I am sure you can find endless videos of her performances going all the way back to the beginning of her career on TV.
I would not worry about accountability.....I am certain Melissa will pursue any and all options available to see that there is justice for Joan IF indeed anyone was responsible; At age 81, it might be hard to prove.

I've been teary eyed for most of the day. The circumstances don't matter to me. None of us will live forever and when it's our turn to go...we go.
And I'll miss Joan so much, since she was always so full of life. It's hard to think of that life being gone.

This procedure is done in this setting. Assuming there were no indications of any health problems, and I would think there weren't, since she was working even the night before, there is no reason just due to her age to do this commonplace procedure elsewhere. I'm sure this office was very much like a thorough clinical atmosphere.Joan only got the best of the best, and I'm sure went to the best places. This wasn't like general anesthesia for a intense, surgical procedure. I had this done twice and was fairly awake during the procedure, more like a twilight sleep.
This heart attack may have occurred at the same time if she had been at home anyway.
I really don't think it can truly be blamed on the procedure. Joan has had heart problems for quite some time , that's not a secret she kept from the world. At her age, this should not come as a total surprise.
If it's your time to go, it's your time to go.
I will miss her so much, I am in severe mourning.
My heart breaks for Melissa, "Coop", David Dangle, all of her employees, family and friends.
I'm not over Robin Williams yet, now her.

I remember seeing Joan Rivers on the Tonight show with Johnny Carson. I remember when she married and when she lost her husband years after. I remember Joan when she had Melissa. She was a funny lady and led a very full life. I am glad she was able to take her grandson to Europe this past summer.
May she rest in peace and be reunited with Edgar, her husband. May Melissa find the strength to deal with her grief and pain of loss and may she find peace of mind as time goes by.

If the clinic is being investigated that is just to make sure proper procedures were followed. Nothing suspicious about that.
Joan's best friend spoke briefly after visiting her in the hospital about how she and he met for dinner at midnight the night before this happened.
Perhaps she shouldn't have eaten anything after midnight?

We don't know what "went on in that clinic". To throw blame at medical people who were caring for her in what should have been a very simple procedure - to accuse them of medical malpractice - when you have no knowledge of the whole situation is beyond reproach. Joan's heart stopped. When you're 81 years old, having your heart stop beating is not beyond the realm of possibility. It happens to wonderful people every day. My Mom died in my arms on my 40th birthday just as the kids were lighting my candles. It could happen to you or me tomorrow. The trick here is to be prepared - I think Joan was.
Another thought - she loved to work. And she was blessed to be able to work right up until mere hours before her soul departed this good earth. I can't imagine that she would have wanted it any other way.
Please reserve your cruel comments about the medical professionals and your thoughts that "it should never have happened" until someone with knowledge and authority tells us their findings. Clearly you are upset, but to point fingers at medical professionals and say you are certain of medical malpractice is just plain wrong. God decided he needed her more than you and I. We have to accept that.
Very well said Perkup. Joan was not in perfect health, no one is at 81. She was at the doctor's afterall. To signup & post an accusation like that is irresponsible and thoughtless. I just watched a clip of Joan & Melissa where she was telling her if she died tomorrow her life had been so full. It can happen to anyone and does everyday, they just aren't celebrities so no one hears about it. My best friend got up to go to the bathroom at age 29 and was found dead on the floor in the morning. I loved Joan's humor & tuned in to watch her on FP every Friday night. I have a cute story of my niece being taken to her home in Los Angeles when she was young. Her babysitter was also Joan's dog groomer. One day the sitter took her with her and she got to play with Spike & the others. Joan asked if she would like a sandwich. She said yes (thinking she'd get p&j) and Joan brought her a crabmeat sandwich on a croissant. She was a lovely & caring woman off camera and will be truly missed.
