Received Dell Streak 7 - How to activate T Mobile card?

I have my T Mobile card inserted in the tablet but can not get activated. I entered the PIN and now need the PUK code. Chatted with TMobile which told me the card was not registered and would need to activate at a Tmobile store. I don't live by a TMobile store. Help! Where do I get the PUK code to complete set up on SIMs card to use TMobile card?
