Pop ups and a QVC survey showing up?

I just started getting pop-ups and a QVC "survey" which I think is bogus. How do I get rid of the pop-ups? Has anyone else received the survey which opens in a new window when you are in QVC land?

This was happening to me about a year ago. It was not just a survey though, there was also an annoying ad that "hid" itself under my tool bar whenever I logged onto Qvc and would pop out every time I changed screens. I don't think the survey is actually from Qvc

Ladybug, I didn't think the survey was from QVC. I think I solved the problem by restoring my computer to an earlier date.
