I'm thinking Home Reflections too.

 

There might have been something similar from Candle Impressions for a very brief time too.

I found some when I did a Google search. They look the same? I searched Pacific Accents etched glass candle holders & clicked on Images on Google & found some that look like it. Looks like it may have been sold under different names. They have them on Wayfair also on Wal-mart under another seller with different colors.
