Any tips for how to handle an exchange of an item for the same item, different size? Does it work well just to indicate the new size on the return form (box in lower left)?

When this has happened to me, and I can be pretty sure that another size will be right, I usually bite the bullet and make a new order instead of waiting for my return to get processed and the new item shipped.
That could take several weeks easily!
If I really want the item I just pay the extra shipping.
If you can live without the new size for a while, and are willing to take the chance that by the time they get to your order it won't be sold out, then you will save new shipping costs by checking the exchange box.
I taped a printed notice to a return stating in large type that I wanted a size exchange so it wouldn't be missed. I got my exchange, but it took almost a total month.

You pay return shipping and the new item shipping to you. I would order the new item now in case it isn't in stock in a couple weeks. The only benefit to waiting for the return to process and the new item to ship is you will be out the cost of the item x2. To me, it doesn't matter since it's a short enough time.
