I went to high school with a girl, she has since passed away so I don't mind sharing her name.  Her name was Wilmeta (pronounced like wilMEETa) Roach.

There is an eye specialist in my city named Dr Blinder. The worst name I ever heard was  in a waiting room when the nurse called for Toilette who turned out to be a sweet looking little girl with her mom.
