Training Question - 15 wk old puppy ?

We have been training our Puppy since we brought him home at 9 wks, and is good at going when we take him outside. EX: always take him out when take him out of crate, when he wakes, after he plays,... He just doesn't seem to connect that he needs to go to the door and tell us. He will occassionaly go to potty in the kitchen or greatroom. We also limit the areas, and watch him. Has anybody had success with a bell? They are difficult to find and pricey.
