Me:
Amazon: a rain jacket for my dog that was too big
Amazon: a CD that was damaged

Actually I returned two items I purchased from the Q !!! In fact whenever I have something to return it is a Q product/item. When I shop at stores I see what I am buying in person and rarely have to return it.

I returned three boxes of Supplements for my dog who passed last January, 2020. They were on auto ship and were sent out right before she died. They were from Amazon and I returned them at Kohl's.
In 2019, I returned a Dooney I bought on I Love Dooney website because it went on sale for $90 less the day after I ordered it. They would not do a credit, so I bought another one at the sale price and when they both arrived on the same day, I returned one and got my money back for the full price one. PayPal returned my shipping costs.

Didn't actually have to return the items, just got refunds. One was defective media on eBay and the other was a vandalized cookbook, also on eBay. Pages torn out. I bought another to replace it and was amazed to find the same thing--whole pages ripped out. I have thousands of cookbooks and have never seen that before. Someone probably taught a cooking course, tore out recipes for the students, and dumped the books on the secondhand market. I now know that I can only buy that book if it's brand new, but enough sellers have the equipment to shrink wrap and sell a used book as new that I'm holding off.
That's not "stores," though. I can't remember the last time I returned something to an actual store. I hate making returns, so I'm pretty careful about what I buy in person.

Hmm! I got a bad Bluray DVD that would not play in any of my 3 players. Amazon sent me a new one and told me to keep the 1st one. I am guessing it cost them more for return shipping than the DVD was worth. Sooo! Guess that don't count as a return.
Other than that? Don't remember any actual returns.

Can't remember the last time I returned anything. Most of my purchases are from vendors I'm familiar with. And if it's a clothing item, rather than return it, I just donate it.
Would never buy big tickets items from the shopping networks. Have to see it in person.

Most QVC fashion designers I purchase (Linea, Bob Mackie, LOGO, Denim & Co, fit me well, so I rarely return clothes.
The only thing I ever really return is fine jewelry. I cannot always judge a piece until I get it home. I am very picky about diamond sparkle, etc., and if I am not happy I will return pretty fast. I buy much less jewelry on QVC because the diamond quality is so basic. Judith Ripka is about the only designer that tempts me, due to her fancy gold designs. I always wait for the big jewelry sales on QVC to get the best prices.
Right now I am boxing up a Shop HQ piece to send back, because the day after I bought it they had a 20% off coupon (which are not retroactive). Anymore I wait for the coupons to get the best prices. I just hate to overpay!

I rarely have to return anything anywhere. The last time was 2 Christmases ago my Mother sent a Phalzgraff dinner plate to replace one of my chipped ones from Replacements Ltd. I received a holiday themed plate instead of the standard plate. Both sides said it was the others fault, however, I was able to send the plate in for an exchange. I did have to pay the shipping though. I can't remember the last time I returned anything back to QVC.

1. It had to be spring or early summer of 2019 - a wig to QVC.
2. About three years ago - Mr. Coffee coffee maker to Ace Hardware - brand new out of the box - it didn't brew.
