Someone please explain why I have to sign up for Medicare now?

This whole Medicare/Medicaid issue is a total mystery to me. I received a notice from SS earlier this year telling me that I have to sign up (with whom? where? how?) for Medicare in the next couple of months.

Very confused on this issue.
